Mookai Rosie Bi-Bayan is committed to providing quality care and services for our mob. We value your feedback and encourage you to let us know how we are meeting your expectations. It is an opportunity for us to understand what we are doing well and where we may need to make changes and improvements.

 

How to provide feedback or make a complaint:

  • In person – come speak with our Practice Manager at the Clinic, or the Health Program Manager at our Accommodation Facility
  • By completing the Patient Feedback Survey located at Reception
  • By filling out our online Complaints Form
  • By filling out our online Compliments Form
  • By calling us on (07) 4081 7500

We will respond to complaints within 30 days. You have the right to remain anonymous when submitting this form. All forms received, whether anonymous or not, are managed in line with our Feedback and Complaints Policy. It is important to note that where an anonymous form is received, we cannot update you of outcomes.

All complaints are processed in accordance with our Feedback and Complaints Policy and Procedure.

If you are unsatisfied with the outcome of a complaint, or with how it was managed, you can refer the matter to the Chief Executive Officer, Chairperson of the Board and/or Board of Directors.
